  I hereby submit my application for membership in the CHESTER WASHINGTON GOLF CLUB subject to the acceptance by the Board of Directors.  Enclosed are the appropriate fees to cover memberships for one (1) calendar year from date of enrollment.  I agree to abide by the rules set forth in the Club's Constitution and By-laws.

Regular Membership: $100.00   ($105.00 on line)
Senior Membership:    $70.00     ($75.00 on line)

Once you submit this form, you will notified of your membership and once approved, you will need to send your membership fee to:
